IMPORTANT NOTE The internal questionnaire that we provide below is not the official DS-160 form, it is only an internal format to facilitate the filling of the DS-160 form if you wish to utilize our immigration services.

On the day of your appointment at MEXUS, either in person or by phone, it is necessary that you have your Mexican Passport with a minimum validity of 6 months or if it is a renewal process, please present your Visa that is still valid or expired to renew.

Fill out the form with your answers

* If the person who will pay for the trip is someone other than the applicant, please provide the following information: Full name, mailing address, and phone number of the person with whom you will be staying during your stay in the United States. If you have traveled to the United States before, please provide your five most recent visits, day, month, year, and the duration of those trips. In case you are traveling with relatives, please provide the name and relationship of your companions. If your Visa or Passport was lost or stolen, please describe what happened, on what date. Send us data or photostatic copies of the lost documents and the report you made with the corresponding authorities. Let us know if you have any criminal records in the United States, as well as any prior Visa rejection. Provide a copy of your records, if you have any documents. Describe important information such as places, events, dates, and attach them on a separate sheet as part of your testimony. Fill out the full name, place, and date of birth of both applicant's parents. Enlist your main activities in the past five years, such as work, study, or something else. Company name or university, address, phone number, direct supervisor's name, position, worked or study periods with day, month, and year required. Provide your income if you are an employee and a description of your activities. If you have higher education than a middle school or university degree, provide school name, address, phone number, and periods with starting and finishing dates, months, and years of study.
